Two young deaths at Scottish prison 'might have been avoided', inquiry finds
21-year-old Katie Allan was found dead in her cell in June 2018. Four months later, 16-year-old William Lindsay also died in custody, and an inquiry has found "systemic failures" contributed to their deaths.
An inquiry found that 'systemic failures' contributed to their deaths and made 25 recommendations to try to prevent a similar tragedy.
